The 1095 tax form verifies health insurance coverage for calendar year 2015 and is reported to the Internal Revenue Service (IRS) for you. The form is used to verify that you and your tax dependents had minimum qualifying health insurance coverage during the year. You may receive more than one 1095 form based on the type of health care coverage you and your dependents had in 2015 (for example: Medicaid/FAMIS, Employer sponsored, or Federal Marketplace coverage).
If you, your spouse, or one of your children were enrolled in a Medicaid or FAMIS program meeting the minimum health insurance coverage requirements in 2015, you will receive a 1095-B form from the Commonwealth of Virginia. The 1095-B form may also be used for anyone covered by individual market insurance, Small Business Health Options Program (SHOP), some employer sponsored coverage, and certain other coverage providers.
The Commonwealth of Virginia should mail the 1095-B forms by February 5th, 2016. The Commonwealth will also send this information directly to the IRS for you. You will not need to attach the 1095 form to your federal income tax return, although you should keep the 1095 form with your tax records. (Note: You will not receive a 1095-B form for enrollment in the following Medicaid programs: Plan First, GAP, Medicaid Emergency Services for dialysis and QMB premium payments. These programs do not meet the minimum health insurance coverage requirements.)

If you had health insurance coverage through your employer in 2015, you may receive either a 1095-B or 1095-C form depending on the type of coverage. Ask your employer for a copy of your 1095 Proof of Health Insurance form.
